The U.K. has a division called the Marine Accident Investigation Branch. It investigates nearly all vessel sinkings, or serious accidents, even when there is no loss of life. Happily, in case #19, the one we are looking at today, the occupants of the boat were rescued just as the boat dropped below the waves.
